1095-C Information

IMPORTANT NOTICE REGARDING 1095-C

Under current IRS guidance and the Paperwork Reduction Act, employers are no longer required to automatically mail Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) to employees.

Form 1095-C provides information about health coverage that was offered to you during the prior calendar year. While this form is not required to be attached to your federal income tax return, you may still need it for your personal records.

If you would like to receive a copy of your Form 1095-C, you may request one at no cost by contacting:
